Preventicus and GCCCF join forces

Preventicus and the Global Clinical + Care Coordination Forum (GCCCF) share a joint vision: Reinventing the continuum of care for advanced patient-centricity, better prevention, and improved outcomes. By leveraging the potentials of digital technologies and telemedical services. By implementing integrated care programs. By smart orchestration of these components to ensure the sustainability of our healthcare systems. Therefore, we agreed a close collaboration with the GCCCF.

The GCCCF offers an international platform for interdisciplinary teamwork of healthcare providers, academia, policy makers, industry, and civil society, seeking to drive the transformation of care delivery. In this context, the GCCCF runs an international program of roundtables which have been held, to date, in London, Paris, Hamburg, Moscow, Warsaw, Bucharest, Krakow, Copenhagen, and Belgrade. International healthcare leaders are regularly invited to these events to discuss the issues and opportunities faced by health organizations, to challenge current practice and debate the innovations needed to make a difference. See www.gcccf.net for details.

The partnership between Preventicus and the GCCCF started back in 2018, when Preventicus became the official cooperation partner of the GCCCF’s annual conference “CARE 2025”.  This year’s CARE 2025 will take place on November 6 in Hamburg, Germany. So, please save the date and visit our session on the Preventicus Managed Care Program for stroke prevention which covers the complete care path: from large-scale screening, continuous monitoring, and precise diagnostics to adequate treatment.
